What is the Well-Being HUB project?

​​

The Well-Being HUB was launched in 2020 as a way for Nova Scotia Health (NSH) and community organizations to work together on identifying and filling gaps in services offered through the Eastern Shore and Musquodoboit Valley. Our network of health and social care providers work together to connect community members with programs, resources and services, offering wrap around supports.

 

We support people who are aging in place, transitioning to supportive care, and providing unpaid care to their loved ones. Our goals are to support community members remaining in their homes and communities longer and to ensure unpaid care partners can access supports close to home.

 

HUB offerings include:

  • the Well-Being HUB Navigation Service

  • Visiting Buddies, a volunteer visitor program for isolated older adults

  • Programming and information sessions

 

VISION

A supportive community where adults can transition through life with dignity, accessing the right care and services at the right time.

MISSION

To create a network of community and health providers who will work collaboratively to provide the right care and supports empowering adults and their care partners as they move through life transitions. The network will work to navigate, educate, coordinate and facilitate access to existing programs and services and create new programs to fill identified gaps.

Programming

Well-Being HUB coordinates our own and highlights our partners' programming on topics related to aging in place, transitioning into supportive care, and unpaid caregiving.

  • Artsy Fartsy Paint Event! An intergenerational creative gathering

  • Panel Discussion on End of Life Planning: Nova Scotia Health (NSH)'s Palliative Care Team, Bereavement Services and the MAID program

Navigation Service

 

Our Well-Being HUB Navigator, Michelle Williams, offers support to community members through the Eastern Shore and Musquodoboit Valley who are:

  • aging in place

  • transitioning into supportive care

  • providing unpaid care to loved ones.

She can provide information on services & practical supports while helping you access resources, navigate systems, and connect with necessary care.

Michelle is available in person on:

  • Tuesdays from 9:30 am - 2:30 pm at the NS Works office in Middle Musquodoboit 

  • Wednesdays from 9:30 am - 2:30 pm at The Old School Community Gathering Place in Musquodoboit Harbour

  • Thursdays from 10 am - 2 pm at the NS Works Office in Sheet Harbour.

Visiting Buddies Program

 

We're on the search for volunteers and participants for our Visiting Buddies Volunteer Visitor Program!

​

Volunteers are paired up with an older adult who could use some company, or whose care partner needs respite. The volunteer visits their "match" in their home or in the community for about two hours per week. Matches last for three months, with the option to extend if resources are available.

 

Visits are social, and can include outings, doing an activity at the client's home, providing company and help for errands, some navigation to resources, hospital visits, etc. Volunteers and their "matches" decide together on a visit schedule that works for both, so the commitment is flexible. Volunteer Orientation and regular training opportunities are offered.
